Form of ID, such as driver’s license or valid state ID;

Any relevant copies of X-rays, MRIs, recent blood work, or other reports you want us to have.

We encourage each person who comes to our office to call their insurance provider to find out information specific to your plan, including the details of your benefits. Our office will also verify and review your health insurance benefits with you.

Copays are due at the time of your visit. Ask us if you are interested in pre-paid plans that do not go through your health insurance provider.
